You will need to pay any deductible and fee co-payments the day of your appointment with cash, check, CareCredit, or any major credit card. If your insurance company has denied or not paid your account in full in 45 days, the balance of your account will then become your responsibility to pay within 15 days. Please be aware that some, and perhaps all, of the procedures may be “non-covered” services and not considered necessary under your insurance program. Regardless of the insurance company’s determination of usual and customary rates or amount of assignment, you are required to pay
